Job summary

Lumicera is hiring a part-time Pharmacist, Patient Consultation Services, to perform telephonic clinical consultations for specialty medications. Coverage includes two main models: 2 x 10-hour shifts per week (Mon and another weekday, CT times) or 5 x 4-hour shifts per week (Mon-Fri, CT times).

The role conducts patient assessments, reconciliation, education, and medication management across patients, caregivers, and healthcare teams, ensuring safety and adherence.

Qualifications

• ACPE-accredited degree (PharmD or B.Pharmacy) is required.
• Active pharmacist license in the state of residence or reciprocity plan.
• Experience counseling patients and documenting interactions in pharmacy settings.

Responsibilities

• Conduct telephonic clinical consultations to support safe use of specialty meds.
• Perform patient assessments and medication reconciliation to identify barriers.
• Provide education on disease state management, administration, and side effects.
• Answer clinical questions from patients and caregivers to aid decisions.
• Escalate clinical concerns to maintain continuity of care and safety.
• Document consultations and interventions in pharmacy systems per policies.
