Job summary

Lumicera is seeking a Pharmacist, Patient Consultation Services to join our team in a part-time capacity. Two primary coverage models are offered: 2 x 10-hour shifts weekly (Mondays and another weekday) and 5 x 4-hour shifts (Mon–Fri).

This role focuses on telephonic clinical consultations for specialty medications, patient assessments, and education on medicine use and adherence. The ideal candidate will hold an ACPE-accredited degree, be licensed as a pharmacist in the state, and have

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a bachelor’s degree in Pharmacy or PharmD.
• Current, unrestricted pharmacist license in the state of residence or reciprocity within 4 months.
• Experience counseling patients and caregivers regarding medication therapy and disease state management.
• Experience documenting patient assessments, interventions, and medication-related recommendations.
• Experience evaluating medication regimens to identify potential drug interactions, adherence barriers, and other concerns.
• Participate in, adhere to, and support compliance program objectives.
• Ability to consistently interact cooperatively and respectfully with other employees.

Responsibilities

• Conduct telephonic clinical consultations to support safe and effective use of specialty medications.
• Perform patient assessments and medication reconciliation to identify barriers and opportunities for clinical intervention.
• Provide education on disease state management, administration, storage, treatment expectations, and side effect mitigation.
• Answer clinical questions from patients and caregivers to improve understanding of prescribed therapies.
• Identify patient-specific clinical concerns and escalate findings to promote continuity of care and safety.
• Document consultations, assessments, interventions, and recommendations in pharmacy systems per policy.
• Maintain productivity and service level standards through timely completion of consultations.
• Support quality improvement initiatives through reporting of medication errors and safety concerns.
• Maintain current knowledge of specialty medications, guidelines, and emerging therapies.
• Adhere to federal/state laws, regulations, and organizational policies.
