Job summary

Lumicera is hiring a part-time Pharmacist, Patient Consultation Services to support telephonic clinical consultations with specialty pharmacy patients. The role involves patient assessments, medication reconciliation, education on medication use and disease state management, plus collaboration with caregivers and healthcare professionals to ensure safe, effective therapy.

Two scheduling models offer flexibility: 2x10-hour shifts weekly or 5x4-hour shifts, with potential for additional coverage.

Qualifications

• Graduate of an ACPE-accredited School of Pharmacy with a bachelor’s degree in Pharmacy or PharmD.
• Current, unrestricted Pharmacist license in the state of residence or reciprocity eligibility.
• Experience counseling patients and caregivers on medication therapy and disease state management.
• Experience documenting patient assessments and interventions in pharmacy systems.
• Ability to collaborate with healthcare teams and support program objectives.

Responsibilities

• Conduct telephonic clinical consultations to support safe, effective use of specialty medications.
• Perform patient assessments and medication reconciliation to identify adherence barriers and concerns.
• Provide education on disease state management, administration, storage, treatment expectations, and side effects.
• Answer clinical questions from patients and caregivers to support informed treatment decisions.
• Document consultations and clinical recommendations in pharmacy systems per policies.
• Maintain productivity, quality, and service levels; contribute to patient safety and quality improvement.
