Job summary

Lumicera is seeking a Pharmacist, Patient Consultation Services, to provide telephonic clinical consultations for specialty medications. This part-time role supports two scheduling models: 2×10-hour shifts per week (Mon plus another weekday, CT hours) and 5×4-hour shifts (Mon-Fri, CT hours).

The pharmacist will assess patients, reconcile medications, educate on use and disease management, and collaborate with healthcare professionals to ensure safe, effective therapy.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a bachelor’s degree in Pharmacy or PharmD.
• Current, unrestricted licensure as a Registered Pharmacist in the state of residence, or the ability to gain reciprocity within 4 months.
• Experience counseling patients and caregivers regarding medication therapy and disease state management.
• Experience documenting patient assessments, clinical interventions, and medication-related recommendations in pharmacy systems.
• Experience evaluating medication regimens to identify potential drug interactions and adherence barriers.
• Ability to participate in and support compliance program objectives and collaborate with team.

Responsibilities

• Conduct telephonic clinical consultations to support safe and effective use of specialty medications.
• Perform patient assessments and medication reconciliation to identify adherence barriers and drug interactions.
• Provide education on disease state management, medication administration, storage, expectations, and side effect mitigation.
• Answer clinical questions from patients and caregivers to aid informed treatment decisions.
• Identify patient-specific concerns and escalate findings per processes to promote continuity of care.
• Document consultations, assessments, interventions, and recommendations in pharmacy systems per policy and regulatory requirements.
• Maintain productivity, quality, and service level standards through timely completion of consultations.
• Support quality improvement initiatives by reporting medication errors and safety concerns.
• Maintain current knowledge of specialty medications and emerging therapies relevant to patient populations.
• Adhere to federal and state laws, regulations, accreditation standards, and organizational policies.
