Job summary

Qualifications

• ACPE-accredited Pharmacy degree required.
• Current, unrestricted Pharmacist license in the state of residence or ability to obtain reciprocity within 4 months.
• Experience counseling patients and caregivers regarding medication therapy and disease state management.
• Experience documenting patient assessments and interventions in pharmacy records.

Responsibilities

• Conduct telephonic clinical consultations to support safe and effective use of specialty medications.
• Perform patient assessments and medication reconciliation to identify adherence barriers and interactions.
• Provide education on disease state management, administration, storage, expectations, and side effects.
• Answer clinical questions from patients and caregivers to support informed treatment decisions.
• Document consultations and recommendations in pharmacy systems per policies and regulatory requirements.
• Maintain productivity and service standards; support quality improvement initiatives.
