Job summary

ConnectMed360 is seeking a Patient Support Case Manager to join our team. This role offers Monday–Friday hours with multiple EST options and can be fully remote.

The position provides quarterly incentives and significant growth opportunities as you support patients, providers, and caregivers through enrollment and therapy access. You will be the primary contact for patients across the journey, delivering program-approved resources and education to ensure timely treatment initiation and

Qualifications

• Minimum of 5 years of pharmacy experience is required.
• Associate degree or equivalent certificate in pharmacy/healthcare preferred.
• Certifications such as PACS/CHES/CCM are advantageous.

Responsibilities

• Serve as the primary contact for patients, caregivers, and providers throughout the patient journey.
• Support pre- and post-prescription enrollment, access, and program processes.
• Educate patients with approved materials, scripts, and FAQs to aid treatment initiation and continuation.
• Maintain accurate patient records and coordinate with the client’s teams and pharmacies.
