Job summary

RareMed is seeking a Case Manager in a phone-based, patient-facing role supporting patients and healthcare providers through access and reimbursement. The position follows a dedicated 11:30am-8pm EST Monday-Friday schedule and is hybrid with in-office Pittsburgh, PA reporting requirements.

The CM communicates case status, coordinates PA and appeals, verifies benefits, and ensures timely submission of payer documentation while maintaining compliance and patient privacy.

Qualifications

• High school diploma plus 2+ years recent case management experience
• Previous 2+ years in a pharmacy, healthcare setting, and/or insurance background
• Strong interpersonal and written communication skills
• Empathy and customer service focus
• Ability to manage caseload and time effectively

Responsibilities

• Serve as primary point of contact for patients and HCP offices regarding case status and documentation needs
• Conduct inbound/outbound calls to provide updates and ensure first-call resolution
• Coordinate access services across multiple functions including PA, appeals, and benefits verification
• Maintain accurate documentation per SOPs and program guidelines
• Support field teams, providers, and patients to ensure smooth referral processing and timely case resolution
