Job summary

ConnectMed360 is hiring an Oncology Case Manager to work remotely, primarily Monday–Friday, with phased schedule changes and EST alignment. The role focuses on ensuring patient access to oral oncology medications, coordinating with prescribers, insurers, and care teams to streamline reimbursement and coverage.

The position offers incentive bonuses and opportunities for growth, with a salary starting at $60k+ depending on experience.

Qualifications

• Required: High School Diploma or equivalent.
• Desired: Bachelor’s or associate degree OR equivalent program in healthcare field.
• Certifications such as PACS/ CCM/ CHES are a plus.

Responsibilities

• Verify patient benefits and coverage including prior authorization and formulary considerations.
• Facilitate prior authorization and appeals with clinical documentation guidance.
• Educate eligible patients about programs and resources; enroll in assistance programs where applicable.
• Perform patient outreach to ensure prescription fulfillment and adherence.
• Maintain documentation in patient profiles and communicate updates to stakeholders.
• Coordinate with prescribers, pharmacies, and care teams to ensure timely therapy access.
