Job summary

Devoted Medical Group is seeking a Social Work Case Manager to support high-needs patients within a collaborative Complex Care Department. You will coordinate services, develop care plans, and coach patients and caregivers to improve well-being and access to resources.

The role emphasizes population-specific support, interdisciplinary teamwork, and ongoing engagement in care planning, with opportunities for remote work and flexible scheduling where appropriate.

Qualifications

• MSW degree required and active state license.
• Minimum 3 years of experience in medical social work, case management, or care coordination.
• Bilingual in Spanish preferred.
• Healthcare experience preferred (Medicare Advantage a plus).

Responsibilities

• Establish trust with patients telephonically and virtually.
• Collaborate with interdisciplinary team to ensure cohesive, patient-centered care for high-needs patients.
• Identify social barriers and coordinate services to maximize care impact.
• Facilitate end-of-life discussions and long-term care planning when appropriate.
• Coordinate care between behavioral health team and other stakeholders.
