Job summary

ConnectMed360 is seeking a Patient Support Case Manager to join our organization. This role offers flexible work hours and the possibility to work fully remote, with salary starting at $60k and increasing with experience.

You will support patients, caregivers, and healthcare providers throughout the patient journey from enrollment to treatment access. You will deliver educational resources, collaborate with client teams, and help patients initiate and maintain prescribed therapies while

Qualifications

• Minimum of 5 years of pharmacy experience required.
• Preferred: Associate degree or equivalent healthcare program; certifications such as PACS, CHES, CCM are a plus.

Responsibilities

• Serve as primary contact for patients, caregivers, physicians and client teams.
• Support enrollment, access, and ongoing treatment processes.
• Provide program-approved talking points, FAQs, scripts and resources.
• Educate patients and document updates in profiles.
• Coordinate with pharmacies and client department teams to ensure timely therapy.
