ConnectMed360 is seeking an Oncology Case Manager for a full-time remote role. You will work with patients, prescribers, and insurance providers to secure payer coverage and support therapy initiation. Qualifications

• Minimum of 5 years of pharmacy experience
• Minimum of 2 years of patient support program experience
• Bachelor’s/Associate degree or equivalent in healthcare
• PACS/ CCM / CHES certifications preferred
• HIPAA/privacy knowledge essential

Responsibilities

• Verify and determine plan benefits, including prior authorizations and formulary considerations
• Facilitate prior authorization and appeal processes with clinical documentation
• Enroll and educate eligible patients in assistance programs
• Educate patients and coordinate with prescribers and care teams throughout therapy
• Monitor insurance coverage renewals to ensure continuity of therapy
