Job summary

Upstream Rehabilitation is seeking a Full Time Patient Intake Specialist to work remotely from the United States. You will coordinate referrals, gather essential patient information, and schedule appointments with the right clinic for therapy or testing.

You will engage with case managers and partners, review referrals, and help patients access timely care, delivering a seamless, supportive experience in a fast-paced setting.

Qualifications

• High school diploma or equivalent required.
• Experience in phone-based or fast-paced environments.
• Strong communication and relationship-building skills.
• Ability to stay organized and proactive.
• Comfort with learning new tools and systems.

Responsibilities

• Schedule evaluations, treatments, and testing quickly and accurately.
• Review referrals and medical docs to ensure successful appointments.
• Match patients to the right clinic or specialist.
• Serve as main contact for case managers and partners.
• Monitor scheduling activity and identify trends.
• Build relationships with clinics and referral partners.
• Track outreach and help team meet goals.
• Other duties as assigned.
