Job summary

The CORE Institute is seeking a remote Orthopedic MSK Digital Care Team member to support daily operations, care coordination, and documentation. You will work with Care Managers, clinicians, and patients to promote timely, quality outcomes from a home-based setting.

Ideal candidates have 3+ years as CNA/MA, plus experience in medical offices and a familiarity with value-based care. Strong communication and organizational skills are essential for success in this fast-paced remote role.

Qualifications

• Minimum three years of certified nursing assistant or medical assistant experience preferred.
• One year experience in a medical business office, physician’s office, or hospital environment preferred.
• Experience in value-based care / population health preferred.
• Experience in orthopedics or musculoskeletal specialty preferred.

Responsibilities

• Oversee daily inbound calls, fax referrals, emails and tasks for the department.
• Assist Care Managers in performing assigned tasks.
• Ensure patient progress toward outcomes by monitoring care.
• Identify and resolve barriers hindering patient care.
• Improve the quality and completeness of documentation.
• Collaborate with patients, families, nurses, physicians and team members to resolve issues.
• Document in a clear, complete, concise and organized manner.
• Monitor low-risk cases and escalate when needed.
• Adhere to and support the organization’s mission and values.
