Job summary

Icon Health is seeking a Surgical Navigator to support orthopedic patients through the preoperative and postoperative journey. You will advocate and educate patients, helping them navigate care plans, appointments, transportation, and referrals, with a focus on Medicare populations.

The role emphasizes clear communication, care coordination, and collaboration with providers and internal teams, while maintaining HIPAA privacy and high-quality patient experience.

Qualifications

• Minimum 3 years of clinical experience in MSK, orthopedic, or value-based care.
• Excellent interpersonal, written, and verbal communication skills.
• Proven problem-solving ability and initiative.
• Proficient with Zoom, Google Suite, electronic health records (EMR), and call center systems.
• Comfort with ambiguity and experience in fast-paced environments or startups.
• Ability to work remotely in a quiet, secure space.
• Bilingual in Spanish and English is a plus.

Responsibilities

• Conduct outreach to potential surgical patients.
• Administer assessments to identify care goals, barriers, and support systems.
• Act as an advocate and educator for patients, ensuring they understand treatment plans and resources.
• Assist with appointment scheduling, transportation, referrals, and care coordination.
• Support post-operative follow-ups and care coordination to enhance experience and quality of care.
• Track and document patient-reported outcomes (PROs) and feedback.
• Serve as a liaison among patients, providers, and internal teams.
• Promote evidence-based orthopedic care and patient-centered practices.
• Support quality improvement by analyzing trends and addressing care concerns.
• Use EMR and other internal/external systems to facilitate patient scheduling and ensure smooth communication.
