Job summary

The CORE Institute is seeking a qualified team member for a 100% remote Orthopedic MSK Digital Care Team role. You will assist care managers, monitor patient progress, document clearly, and collaborate with patients and clinicians to resolve issues in a timely manner.

Ideal candidates have 3+ years of CNA/medical assistant experience and familiarity with value-based care. This remote position emphasizes strong communication, accuracy, and adherence to HIPAA, supporting a patient-centered care

Qualifications

• Medical Assistant/CNA degree or equivalent working knowledge preferred.
• Minimum three (3) years of certified nursing assistant or medical assistant experience preferred.
• Experience in value-based care / population health preferred.
• Experience in orthopedics or musculoskeletal specialty is preferred.

Responsibilities

• Oversee daily inbound calls, fax referrals, emails and tasks for the department.
• Assist Care Managers in performing assigned tasks.
• Monitor patient care to ensure progress toward desired outcomes.
• Identify and resolve barriers that hinder effective patient care.
• Improve documentation quality and completeness.
• Collaborate with patients, families, nurses, physicians and team members to resolve issues in a timely manner.

100% Remote Orthopedic MSK Digital Care Team

ESSENTIAL FUNCTIONS

• Assists withoverseeing the daily inbound calls, fax referrals,emailsand tasksforthedepartment.
• Assists Care Managers in performing assigned tasks.
• Ensurespatient is progressingtoward desired outcomes by continuouslymonitoringpatient care through assessment and evaluation.
• Identifiesand resolves barriers that hinder effective patient care.
• Improves quality and completeness of documentation.
• Collaborates with patients, families, nurses, physicians,and other members of the team,building consensus around issuesandappropriately resolvesin a timely manner.
• Documents in a clear, complete, concise, and organized manner.
• Monitors clinicallylowriskcases.
• Advises Care Manager and/or Medical Director of any new and/or concerning clinical issues.
• Institutes necessary actions to promote quality care andappropriate.
• Accepts referrals andidentifiespatients who meet the criteria for care management.
• Adheres to and supports team members in exhibitingHOPCo’svaluesand mission.
• Assistswith program planning, development, and evaluation.
• Performs related duties as assigned. Performs other CIN duties as assigned.

EDUCATION

• Medical Assistant or Certified Nursing Assistantdegreeor equivalent working knowledgepreferred.

EXPERIENCE

• Minimum three (3) years ofcertified nursing assistant or medical assistantexperienceis preferred.
• One-year experience in a medical business office, physician’s office, or hospital environment is preferred.
• Experience in value-based care / population health is preferred.
• Experience in orthopedics or musculoskeletalspecialtyis preferred.

KNOWLEDGE

• Knowledge of the health care field and orthopedics.
• Knowledge of policies and procedures concerning patient care.
• Knowledge of patient confidentiality, state laws, HIPAA regulations, and medical records procedures.
• Knowledge of medical practices, terminology, and reimbursement policies.
• Knowledge of Microsoft Office applications.
