Job summary

Optum Infusion Pharmacy is seeking a Patient Care Coordinator for a per diem remote role across the United States. You will manage the initial patient intake, schedule nursing visits, and coordinate care with nursing agencies while maintaining professional contact with patients and families.

This position offers weekend hours with telecommuting from anywhere in the U.S., and pay ranging from $18 to $32 per hour with a comprehensive benefits consideration.

Qualifications

• 6+ months of customer service or healthcare experience.
• Intermediate level of computer proficiency including Microsoft Word, Excel and Outlook and multiple web applications.
• Must be 18 years of age OR older.

Responsibilities

• Receives and executes the initial patient intake process to schedule nursing services.
• Cooperates with Optum nursing and other providers to assure continuity of patient care.
• Answer telephone calls promptly and courteously.
• Maintain professional rapport with patients and families and document communications.
• Coordinate care with alternate providers and document communications.
