Job summary

Evergreen Nephrology is seeking a Remote Pharmacy Care Coordinator to support a mail-order pharmacy program from home. The role focuses on outbound outreach, enrollment, and EHR documentation, with emphasis on HIPAA-compliant processes and patient engagement.

This contract-to-perm position requires 8:30am–5pm ET availability, CMA/RMA certification, and experience in healthcare or pharmacy support. Compensation ranges from $19 to $23 per hour, with strong benefits and remote work setup.

Qualifications

• Experience in telephonic, call-center, or high-volume outbound outreach.
• Ability to document encounters in EHR/EMR and update records.
• Strong communication skills and patient engagement over the phone.
• Knowledge of HIPAA and protecting patient privacy.

Responsibilities

• Conduct high-volume outbound outreach to enroll patients.
• Deliver messaging using approved scripts to drive enrollment.
• Obtain and document patient consent by phone.
• Make EHR updates based on patient decisions (eRx, pharmacy preference).
• Manage outreach queues and ensure timely follow-ups.
