Thrive2Liv, Inc. in the United States is seeking a Telephonic Outreach Specialist to connect remotely with eligible members and begin the intake process for Thrive2Liv Powered by Philips services. You will conduct telephonic/virtual outreach, collect consent, document information in Commons, and hand off to the care team.

This role supports the care team daily and joins a user group for onboarding. Salary is hourly, $21.73–$26.56, with health, life, retirement, and PTO benefits.

Qualifications

• 1 year of experience in a similar outreach role.
• Excellent telephonic/virtual communication skills.
• Ability to intake and triage information to care teams.

Responsibilities

• Provide telephonic/virtual outreach and engagement for eligible members.
• Collect consent and initiate intake to the care team.
• Document information accurately in Commons and track tasks.
• Collaborate with care teams and participate in onboarding support.
