Job summary

Loyal Source Government Services is seeking 10 full-time Outpatient Medical Records Technician (MRT) Coders to provide remote services to the VA Northeast Ohio Healthcare System in Cleveland, OH. MRT Coders must maintain high accuracy and productivity while delivering compassionate care to veterans, adhering to ICD-10-CM/PCS, CPT, HCPCS and E/M guidelines, and querying providers as needed.

Shifts are Monday through Friday: 6:00 a.m.–6:00 p.m. local time, with competitive hourly pay.

Qualifications

• Must have at least 2 years of continuous professional coding experience in a VA facility.
• Must have 2 years of experience in ICD-10-CM, CPT, HCPCS, and E/M guidelines.
• Completed the baseline ICD-10-CM requirements.
• One current coding certification: AHIMA or AAPC (RHIT, RHIA, CCS, CCS-P, CPC, COC, CIC)

Responsibilities

• Apply coding conventions across inpatient, outpatient, professional services, and surgical encounters (ICD-10-CM/PCS, CPT, HCPCS, E/M).
• Query providers to resolve documentation discrepancies per VHA process.
• Analyze health records to identify diagnoses and procedures for coding.
• Read and understand health record content and disease processes.
• Code across major inpatient, outpatient, and surgical specialties.
• Meet or exceed 25 outpatient/prosthetics services per day.
• Review Service Connection/Service Authority questions.
• Exclude irrelevant symptoms and findings from coding.
• Adapt guidelines when gaps exist.
