Loyal Source Government Services seeks a Surgical Medical Records Technician Coders to provide remote coding support to the VA Northeast Ohio Healthcare System in Cleveland, OH. Qualifications

• 2 years of continuous professional coding experience in a VA facility.
• 2 years of experience with ICD-10-CM/ICD-10-PCS/CPT/HCPCS and E/M guidelines.
• Baseline ICD-10 requirements completed.
• One current coding certification (AHIMA or AAPC).

Responsibilities

• Apply coding conventions across inpatient, outpatient, professional services, and surgical encounters.
• Query providers to resolve documentation discrepancies per VHA Provider Query Process.
• Analyze health records to identify diagnoses and procedures for coding and to assess documentation adequacy.
• Read and interpret health records, terminology, and pathology.
• Code across major inpatient, outpatient, and surgical specialties including complex cases.
• Adopt health information guidelines and use judgement when guidance is incomplete.
• Meet or exceed daily surgery service encounters per day.
• Review Service Connection/Service Authority questions.
• Exclude non-specific symptoms from coding when not relevant to current management.
