Job summary

Daymark Health is seeking a National Oncology Nurse Navigator to join a integrated care team and deliver telehealth cancer care coordination. You will manage patient needs from screening to survivorship or palliative care, in collaboration with clinicians.

The role requires a BSN, 3+ years nursing with 2+ years oncology experience, multi-state RN licensing, and strong tech skills for Slack, Zoom, Google Suite, and EHR systems. This is a remote, full-time position.

Qualifications

• Bachelor of Science Degree in Nursing required with licensure in all 50 states (multi-state license).
• At least 3 years of nursing experience, including 2 years in oncology nursing.
• Oncology certifications (OCN / AOCN / AOCNS / CCM) preferred or to be obtained within 2 years.

Responsibilities

• Provide direct patient care through virtual platforms to support oncology patients.
• Triage symptoms and respond to urgent clinical needs.
• Coordinate care and identify additional patient needs across the care continuum.
• Educate patients about diagnosis, Daymark Health offerings, and next steps.
