Job summary

Daymark Health is seeking an Oncology Nurse Navigator to join our integrated Care Team. You will provide triage, case management, and care navigation for oncology patients via phone and virtual platforms, collaborating with Medical Director, Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker.

The role is remote, full-time, with hours Monday–Friday 8:30 AM–5 PM Eastern. Requires BSN and RN license (RI licensure or eligible), 3+ years in nursing (2+ in oncology);

Qualifications

• BSN with an unrestricted RN license and RI licensure or eligibility.
• 3+ years of nursing experience, including 2+ years in oncology.
• Strong communication and care coordination skills.

Responsibilities

• Provide direct patient care via virtual platforms and phone, with timely clinical escalations.
• Triage symptoms and manage care navigation during the oncology care journey.
• Coordinate with the clinical team to align on care protocols and treatment plans.
• Educate patients on diagnoses, Daymark Health services, and next steps.
