Job summary

Daymark Health is seeking an Oncology Nurse Navigator to join our integrated Care Team. You will provide triage, case management, and care navigation for oncology patients over the phone and via virtual platforms.

You will collaborate with the Medical Director, Nurse Practitioner, Social Worker, and Enrollment Specialist to identify optimal care pathways, educate patients about Daymark Health’s offerings, and coordinate services while maintaining thorough documentation.

Qualifications

• BSN required with compact unrestricted RN license; RI licensure preferred or willingness to obtain.
• At least 3 years of nursing experience, including 2 years in oncology.
• Oncology certifications such as OCN/AOCN/AOCNS or CCM preferred or to be obtained within 2 years of hire.

Responsibilities

• Provide consultative clinical support by phone, email, or video.
• Triage symptoms and escalate to NP as needed.
• Coordinate care with medical team and community resources.
• Educate patients on diagnosis, Daymark Health’s offerings, and next steps.
• Build trusting relationships with patients and families.
• Ensure adherence to documentation standards and workflows.
