Job summary

Daymark Health Inc. seeks an Oncology Nurse Navigator to join the integrated Care Team.

You will provide triage, case management, and care navigation for oncology patients, delivering care over the phone and via virtual platforms to improve quality of life. You will work with a Medical Director, Nurse Practitioner, and Social Worker to coordinate care from screening through survivorship, ensuring compassionate, patient-centered service and adherence to clinical protocols.

Qualifications

• BSN required and unrestricted CA RN license.
• Minimum 3 years nursing experience, including 2 years in oncology.
• Oncology certifications such as OCN / AOCN / AOCNS or CCM preferred or to be obtained within 2 years.

Responsibilities

• Provide direct patient care via phone, email, or video.
• Perform triage and symptom assessment with escalation as needed.
• Coordinate care and referrals, discharge planning, and nutritional support.
