Job summary

Daymark Health Inc. is seeking a National Oncology Nurse Navigator to join a virtual, value-based oncology care team. You will perform triage, care coordination, and patient education by phone or video, collaborating with physicians and social workers to optimize treatment pathways.

You will operate in a fully remote setting with business hours Mon–Fri, 8:30 AM–5:00 PM in your time zone, and compensation ranging from $85,000 to $100,000 annually depending on location and experience.

Qualifications

• Bachelor of Science in Nursing required
• Active multi-state RN license (Nurse Licensure Compact) and licensure in non-compact states (CA, NY, IL)
• 3+ years of nursing experience with at least 2 years in oncology
• Oncology certifications such as OCN / AOCN / AOCNS / CCM preferred or to be obtained within 2 years

Responsibilities

• Provide direct patient care via virtual platforms.
• Deliver care coordination and triage for oncology patients.
• Educate patients about diagnosis, Daymark Health services, and next steps.
• Build rapport with patients and care networks with empathy and active listening.
• Collaborate with Medical Directors, Nurse Practitioners, Social Workers, and Health Partners to ensure aligned care.
• Adhere to Daymark policies, documentation standards, and optimize use of systems and tools.
