Job summary

Daymark Health Inc. is seeking an Oncology Nurse Navigator to join a unified care team. This remote role supports oncology patients via phone and virtual platforms, triaging symptoms, coordinating care, and educating patients about Daymark’s services.

You will work closely with a Medical Director, Nurse Practitioner, Social Worker, and Enrollment Specialist to ensure efficient care pathways and compassionate patient experiences.

Qualifications

• Bachelor of Science in Nursing is required.
• Compact unrestricted RN license (or ability to obtain) and licensure in Rhode Island or willingness to be licensed in RI.
• At least 3 years of nursing experience, including 2 years in oncology.
• Oncology certifications such as OCN, AOCN, AOCNS, CCM are strongly preferred or obtainable within 2 years.

Responsibilities

• Provide direct patient care through virtual platforms and deliver high-quality care to meet each patient's needs.
• Deliver comprehensive support throughout the oncology care journey, from screening to survivorship or end-of-life care.
• Triage symptoms and respond to urgent clinical escalations when required.
• Care navigation including referrals, nutrition support, discharge planning, and social work coordination.
• Educate patients on diagnosis and Daymark Health’s care offerings and next steps.
• Collaborate with the clinical team to ensure adherence to protocols and high-quality care.
• Maintain documentation and use Daymark’s systems and tools effectively.
