Job summary

Daymark Health Inc. is seeking an Oncology Nurse Navigator to join a value-based oncology care team.

You will deliver direct patient care remotely and coordinate comprehensive care pathways for oncology patients, supporting comprehensive triage, education, and navigation over phone and virtual platforms. You will work with a multidisciplinary team including Medical Director, Nurse Practitioner, Social Worker, and Enrollment Specialist to ensure optimal care, with emphasis on palliative care,

Qualifications

BSN required with compact unrestricted RN license or ability to obtain in California
Minimum 3 years nursing experience, including 2 years in oncology
Strong interest in palliative care and patient education

Responsibilities

Provide direct patient care via phone, email, or video and deliver high-quality oncology care
Triage symptom responses and escalate to a Nurse Practitioner as needed
Coordinate care needs including referrals, discharge planning, and nutritional support
Educate patients about diagnosis, Daymark Health services, and next steps
Build trust with patients and care networks through empathy and active listening
Collaborate with clinical team to ensure adherence to protocols and optimize systems
