Job summary

Daymark Health is seeking an Oncology Nurse Navigator to join our integrated care team. You will provide telehealth-based triage, care coordination, and patient education for oncology patients, collaborating with Medical Directors, NPs, Social Workers, and Enrollment Specialists.

You will support patients through the cancer care journey, from screening to survivorship, delivering compassionate, patient-centered care over phone and virtual platforms. BLS and RI licensure or eligibility required.

Qualifications

• BSN degree required and RN license valid in RI or eligible for RI licensure.
• Minimum 3 years nursing experience, with 2 years in oncology.
• Oncology certifications preferred or to be obtained within 2 years.

Responsibilities

• Provide direct clinical support via phone, email, or video.
• Triages symptoms and coordinates urgent clinical escalations.
• Coordinate care, including referrals and discharge planning.
• Educate patients about diagnosis, Daymark services, and next steps.
• Build relationships with patients and care networks with empathy.
• Collaborate with clinical team to ensure protocol alignment.
• Adhere to policies and document thoroughly using Daymark tools.
