This remote, full-time role focuses on triage, case management, and care navigation for oncology patients, delivering care over phone and virtual platforms. You will work with a Medical Director, Nurse Practitioner, Social Worker, and Enrollment Specialist to identify optimal care pathways, educate patients, and coordinate services.

Qualifications

• BSN degree required and Rhode Island RN licensure.
• Unrestricted RN license or ability to obtain one.
• Minimum 3 years of nursing experience, including 2 years oncology.
• Oncology certifications preferred (OCN, AOCN, AOCNS, CCM) or will be obtained within 2 years.
• Patient-centered care with strong communication.

Responsibilities

• Provide direct patient care via virtual platforms.
• Perform triage and call triage via Symptom Survey.
• Coordinate care, including referrals and discharge planning.
• Educate patients about diagnosis and Daymark's offerings.
• Build rapport with patients, families, and care networks.
• Collaborate with clinical team to ensure adherence to protocols.
