Job summary

Daymark Health is hiring an Oncology Nurse Navigator to join our integrated care team. You will provide direct patient care remotely, triage symptoms, and support oncology patients through their care journey.

You’ll work with a Medical Director, NP, Social Worker, Enrollment Specialist and Health Partners to coordinate care, address social needs, educate patients, and ensure adherence to Daymark’s pathways.

Qualifications

• BSN required with RN license and RI licensure or eligibility
• At least 3 years nursing experience, including 2 years in oncology
• Oncology-related certifications (OCN, AOCN, AOCNS, CCM) preferred or to be obtained within 2 years
• Comfort with Slack, Zoom, Google Suite, and EHR systems
• Ability to establish rapport with diverse populations incl. clinicians, patients and families
• Bias to action and ability to work independently in dynamic settings
• Patient-centric approach and commitment to Daymark's mission
• Strong organizational, multitasking and documentation skills
• Excellent communication and adaptability in changing environments

Responsibilities

• Provide direct patient care remotely via phone, email, or video
• Triage symptom responses and escalate to NP as needed
• Coordinate care, including referrals, nutrition, discharge planning, and social determinants of health
• Educate patients on diagnosis, Daymark offerings, and next steps
• Build strong relationships with patients and care networks through empathy and listening
• Collaborate with clinical team and health partners to ensure quality care and protocol alignment
• Adhere to policies, procedures, and documentation standards; optimize use of systems and tools
