Job summary

Daymark Health Inc. is seeking a Nurse Practitioner or Physician Assistant to deliver high-quality cancer care in patients’ homes and through virtual platforms.

You will join a patient-focused team, triage high-acuity cases, and help implement Daymark’s evidence-based oncology care and navigation model. You will work with our clinical operations group to support symptom management, in-home urgent care, palliative and behavioral health services, while building relationships with patients,

Qualifications

• Active NP or PA with at least 4 years of direct clinical care experience.
• Oncology, home care, palliative or hospice care, or primary care background required.
• Oncology Nursing Society certification and value-based care experience a plus.

Responsibilities

• Provide direct patient care in home and via virtual platforms.
• Triage high-acuity patients and coordinate with oncology practices.
• Support care continuum from active treatment to survivorship or end-of-life care.
• Build trusting relationships with patients and care networks.
• Collaborate with clinical operations to refine and implement the Daymark care model.
