Qualifications

• Active NP or PA with 4+ years of direct clinical care in oncology, home care, palliative, hospice, or primary care.
• Oncology Nursing Society certification and value-based care experience are a plus.

Responsibilities

• Provide direct patient care in home and via virtual platforms.
• Assist in triage for high-acuity patients and coordinate with oncology practices.
• Support patients across the full care continuum from active treatment to survivorship or end-of-life care.
