Job summary

Daymark Health is seeking a Nurse Practitioner or Physician Assistant to deliver hands-on cancer care in patients' homes and through virtual platforms. You will collaborate with a clinical operations team to implement and refine Daymark's evidence-based oncology care model, addressing symptom management, in-home urgent care, and behavioral health needs.

Qualifications

• Active NP or PA with at least 4 years of direct clinical care experience.
• Oncology, home care, palliative/hospice, or primary care experience required; value-based care experience a plus.
• Familiarity with Slack, Zoom, Google Suite and EHR systems is expected.

Responsibilities

• Provide direct patient care in patients' homes and via virtual platforms.
• Assist in triage and management of high-acuity patients with oncology teams.
• Support patient care across active treatment to survivorship or end-of-life care.
