Job summary

Daymark Health is hiring a Nurse Practitioner to deliver compassionate oncology care in patients' homes and through virtual visits. You will lead care delivery with the clinical operations team, triage high-acuity patients, and help implement Daymark's value-based care model in Orange County, CA.

You must be an active NP/PA with 4+ years' direct clinical experience in oncology or related fields, certified as BRN 103/104 in California, and proficient with EHR systems, Slack, Zoom, and Google

Qualifications

Active NP/PA with 4+ years of direct patient care in oncology/home/palliative care.
CA BRN 103/104 certification required.
In-home and virtual care delivery experience preferred.

Responsibilities

Provide direct patient care in home and via virtual platforms.
Conduct assessments, place orders, prescribe medications, document care.
Triages high-acuity patients and coordinates with oncology practices.
Support patients across the full care continuum from active treatment to survivorship or end-of-life.
Build trusting relationships with patients and care networks.
Collaborate to refine and implement Daymark's care model.
