Job summary

Daymark Health is redefining cancer care by delivering hands-on, compassionate clinical care in patients’ homes and through virtual platforms. This Nurse Practitioner role requires independent, player/coach leadership to grow a high-performing clinical team in San Diego and to implement an evidence-based oncology care and navigation model.

Qualifications

• Active NP or PA with at least 4 years of direct clinical care experience (oncology, home care, or palliative care).
• California BRN 103/104 certification required.
• Comfort delivering care in home and via virtual platforms; in-person in San Diego.
• Strong organizational, communication, documentation skills; proficient with Slack, Zoom, Google Suite, and EHRs.
• Bias to action and ability to work in ambiguity; player/coach leadership.
• Valid driver’s license and reliable vehicle for in-home visits.

Responsibilities

• Provide direct patient care in home and via virtual platforms; place orders and document accordingly.
• Triages high-acuity cases; collaborate with oncology practices through daily standups.
• Support patients across full care continuum from active treatment to survivorship or end-of-life.
• Build trusting relationships with patients and care networks through empathy and listening.
• Refine and implement Daymark’s care model to deliver right care at the right time.
