Job summary

ComplexCare Solutions seeks a Nurse Practitioner per diem to deliver in-home and virtual assessments across Middlesex County, MA. You will spend a full hour with members to capture their personal needs, evaluate health status, identify gaps in care, and support care management aimed at keeping patients safe at home and out of the hospital.

Candidates must hold ANCC or AANP certification, an active nursing license, and be able to work autonomously in a remote clinical environment.

Qualifications

• Active un-encumbered nursing license.
• ANCC or AANP certification as a Nurse Practitioner.
• Experience with in-home assessments and care management.
• Ability to work autonomously in remote clinical environments.

Responsibilities

• Conduct comprehensive in-home assessments with patients.
• Document findings and coordinate with PCPs and health plans.
• Identify gaps in care and provide evidence-based recommendations.
• Maintain CMS/DHHS compliance and HIPAA requirements.
• Utilize EHR systems to document assessments and follow-ups.
