Job summary

ComplexCare Solutions (CCS) seeks a Nurse Practitioner per diem to cover Marin County, CA, with a focus on in-home assessments and telehealth options for health plans nationwide.

You will conduct comprehensive member assessments, document findings, and develop evidence-based treatment plans while coordinating with PCPs and care teams. An active NP license, board certification, and HIPAA compliance are required.

Qualifications

• Active un-encumbered license to practice nursing.
• ANCC or AANP board certification as a Nurse Practitioner or Clinical Nurse Specialist.
• Maintains current CPR certification.

Responsibilities

• Perform, document and communicate results of patient assessments.
• Develop evidence-based treatment plans and follow-up timelines.
• Coordinate with PCP and health plan; communicate recommendations.
