Job summary

ComplexCare Solutions (CCS) seeks a Nurse Practitioner for per diem in San Joaquin County, CA, conducting comprehensive in-home assessments and telehealth visits. You’ll document findings, coordinate with PCPs and health plans, and create evidence-based treatment plans for complex chronic conditions.

Per diem role with flexible scheduling; in-home visit pay rates and telehealth compensation are provided. Eligible for company 401K with automatic enrollment and 3% contribution.

Qualifications

• ANCC or AANP board certification as NP.
• Active unencumbered NP license.
• Current CPR certification.
• HIPAA compliance and vaccination/testing requirements as applicable.

Responsibilities

• Conduct comprehensive, accurate and thorough member assessments in home or telehealth settings.
• Document encounters timely and thoroughly in the medical record.
• Communicate recommendations and follow-up plans to PCP and health plan.
• Develop evidence-based treatment plans and manage chronic conditions.
• Escalate emergent situations per policy and professional judgment.
