Job summary

ComplexCare Solutions (CCS) is seeking a Nurse Practitioner to provide per diem, in-home and telehealth assessments from a remote clinical setting. The role covers all zip codes in Madison County, Nebraska, focusing on evaluating health status, gaps in care, and developing care plans to keep members at home and out of the hospital.

You will document encounters, coordinate with PCPs and health plans, and ensure timely follow-up.

Qualifications

• Active NP license in good standing
• ANCC or AANP board certification as NP
• Ability to conduct comprehensive member assessments
• Experience with CMS/Medicaid/Health Plan processes
• Proficient with EHRs and secure messaging

Responsibilities

• Perform comprehensive member assessments in home or virtual settings
• Document encounter findings and care plans promptly
• Coordinate with PCPs and health plans for follow-ups
• Identify and close gaps in care and coordinate referrals
• Maintain HIPAA compliance and accurate medical records
