Job summary

ComplexCare Solutions seeks a Nurse Practitioner to perform per diem in-home assessments for Medicare/health plan members. You will conduct comprehensive evaluations, document findings, and coordinate with physicians and plans to optimize outcomes.

The role emphasizes autonomous practice in a remote/home setting, with documentation and follow-up in a compliant healthcare environment. Prior home health or telehealth exposure is a plus.

Qualifications

• Active un-encumbered nursing license in MA.
• ANCC or AANP board certification as NP in Family, Adult, Gerontology or Emergency.
• CPR certification current and valid.
• Experience with home health or telehealth a plus.
• Familiarity with CMS/Medicaid requirements is beneficial.

Responsibilities

• Conduct comprehensive member assessments in-home or via telehealth.
• Document encounters and care plans per CMS and client requirements.
• Coordinate with PCPs and health plans for follow-up and referrals.
• Address gaps in care and provide patient education on chronic conditions.
• Maintain HIPAA compliance and accurate medical records.
