Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for per diem work to conduct in-home and telehealth assessments for members across various states. You will capture a comprehensive view of each member’s needs, ensuring timely documentation and care planning within the approved guidelines.

Responsibilities include thorough charting, collaboration with PCPs and health plans, and proactive identification of gaps in care while upholding HIPAA and CMS requirements.

Qualifications

• Active unencumbered nursing license.
• ANCC or AANP board certification as a Nurse Practitioner.
• Familiarity with Medicare/Medicaid and health plan benefits.

Responsibilities

• Conduct comprehensive home or telehealth member assessments.
• Document findings, assessments and care plans in the EHR.
• Coordinate with PCPs and health plans; communicate updates and follow-ups.
• Identify and address gaps in care and provide recommended actions.
• Maintain HIPAA compliance and confidential handling of data.
