Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for per diem in California to perform in-home assessments and telehealth when applicable. The role emphasizes comprehensive evaluations, care plan development, and coordination with physicians and health plans to improve member outcomes.

The position requires active NP licensure, board certification, CPR, and adherence to CMS/HIPAA standards, with flexible monthly availability and a focus on delivering high-quality, patient-centered care.

Qualifications

• Active unencumbered nursing license.
• ANCC or AANP board certification as NP.
• CPR certification current.
• Vaccination & preventive testing per policy (Hep B, Flu, MMR, Tdap, Varicella, TB).

Responsibilities

• Conduct thorough member assessments and document findings.
• Maintain comprehensive medical records per CMS/DHHS requirements.
• Communicate results and recommendations to PCPs and health plans.
• Close identified gaps in care and address urgent situations.
• Perform and document Point of Care testing.
• Ensure HIPAA compliance in all duties.
