ComplexCare Solutions is seeking a Nurse Practitioner for per-diem in-home assessments across Douglas County, CO. The role emphasizes comprehensive patient evaluations, chronic care management, and coordination with primary care providers and health plans.

Qualified candidates will hold an NP/CNS certification, maintain CPR, and demonstrate autonomous practice in a remote/home-based setting with strong EHR proficiency and patient communication skills.

Qualifikationen

• Active, unencumbered nursing license and NP/CNS board certification.
• CPR certification maintained per policy.
• Hep B, Flu, MMR, Tdap, Varicella, TB vaccinations required before hire.
• Home health experience is a plus; able to work autonomously in a remote setting.
• Familiarity with Medicare/Medicaid benefit structures and HIPAA rules.
• Strong communication with elderly/chronically ill patients and families.
• Proficient with EHRs, secure messaging and cloud-based documentation.

Aufgaben

• Conduct comprehensive member assessments in home or telehealth settings.
• Document and submit encounter notes in compliance with CMS/DHHS and client policies.
• Identify and close gaps in care and provide evidence-based care plans.
• Coordinate with PCPs and health plans; communicate follow-ups and needs.
• Provide patient education to improve understanding of chronic conditions.
• Escalate urgent issues per policy and professional judgment.
