Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for per diem home and telehealth assessments nationwide. The role covers all zip codes in Carter County, OK, with an emphasis on evaluating health status, gaps in care and care management opportunities to improve outcomes.

Qualified candidates will have an active NP license, board certification, CPR, and the ability to work autonomously in a remote setting. In-home visits, potential telehealth assessments, and 401K eligibility are offered.

Qualifications

• Active un-encumbered license to practice nursing.
• ANCC or AANP board certification as NP or CNS in relevant specialties.
• Maintains current CPR certification.
• Vaccination and preventive testing as required by healthcare policies.
• Home Health experience a plus.

Responsibilities

• Review member population comprehensively and submit encounter documentation.
• Document active medical conditions in the medical record per CMS/DHHS and client requirements.
• Communicate findings and recommendations to PCP and health plan with timelines for follow-up.
• Identify and close gaps in care and provide disease-specific guidance.
• Perform Point of Care testing and document results.
• Maintain HIPAA compliance and confidentiality in all actions.
• Support physician/patient relationships and coordinate additional services when needed.
• Be available for a minimum of 25 slots per month (≈37 hours).
• Adapt duties as business needs change.
