Job summary

Signature Performance in the United States is seeking a Medical Coder specializing in Surgery and Anesthesia to ensure precise E/M ICD-10-CM/PCS, CPT, and HCPCS coding from medical records. Your role will review discharge summaries and physician notes, assign diagnoses and procedures, sequence codes by clinical significance, and support compliant reimbursement under CMS and payer rules.

This is a remote nationwide position with full-time hours and a comprehensive benefits package.

Qualifications

• Minimum 2 years of Medical Coding experience.
• Experience with Professional Fee Coding.
• Experience with EHR systems.
• AHIMA or AAPC credentials include RHIT, RHIA, CCS, CCS-P, CPC.

Responsibilities

• Assign principal and secondary diagnoses and procedures by reviewing documentation.
• Review discharge summaries, notes, radiology, labs, and operative reports to assign codes.
• Assign codes following Official Coding Guidelines and payer regulations.
• Ensure correct sequencing to determine DRG/APC or payment tier for reimbursement.
• Abstract necessary data and correct discrepancies in the medical record and system.
• Identify missing or conflicting documentation and query physicians if needed.
• Meet coding quality and quantity expectations.
