Job summary

Signature Performance is seeking a Medical Coder specializing in Surgery and Anesthesia to ensure accurate coding of E/M, CPT, ICD-10-CM/PCS, and HCPCS data from medical records.

You will review records, assign diagnoses and procedures, ensure compliance with guidelines, and support correct DRG/APC categorization for proper reimbursement. The role is remote with standard business hours.

Qualifications

• Minimum 2 years of Medical Coding experience required.
• Experience with Professional Fee Coding and EHR systems.
• AHIMA or AAPC certifications (RHIT, RHIA, CCS, CCS-P, CPC, CCA) are preferred.

Responsibilities

• Assign principal and secondary diagnoses and procedures by reviewing documentation.
• Review discharge summaries, H&Ps, progress notes, and reports to assign diagnoses/procedures.
• Assign codes following Official Coding Guidelines and compliance with regulations.
• Ensure diagnoses and procedures are sequenced for correct DRG/APC or payment tier.
• Abstract data from records and enter into the system; correct data where needed.
• Identify missing or conflicting documentation and formulate physician queries to obtain clarification.
• Meet coding quality and quantity expectations.
