Cosán Group is seeking a Care Coordinator to join our fully remote US team. You will manage a patient panel, provide health coaching, and drive monthly outreach to close gaps in care.

Ideal candidates hold a CMA/related certification, have 1+ year of clinical experience, and are proficient with EMRs and RPM platforms. A HIPAA-compliant home office and bilingual English/Spanish skills are preferred.

Qualifications

• Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent).
• Minimum 1 year of clinical experience in primary care, hospital, or specialty healthcare.
• Experience with EMRs and strong computer navigation skills.
• HIPAA-compliant home office setup with high speed internet.

Responsabilités

• Manage an assigned panel of 400-450 patients with monthly outreach.
• Conduct 50-60 daily outbound calls for care plan support and health coaching.
• Collaborate with clinical teams to address clinical and social needs.
• Document all patient interactions in real-time on the care coordination platform.
