Job summary

Cosán Group is seeking a compassionate Care Coordinator to manage a patient panel and deliver health coaching from a fully remote home office in the US. You will perform monthly outreach, support care plans, and collaborate with clinicians to close gaps in care.

The role requires CMA/RMA/CCMA/NCMA/CMAC certification, 1+ year clinical experience, EMR proficiency, and HIPAA-compliant setup with reliable internet. Bilingual English/Spanish preferred, with 100% remote, weekday schedule.

Qualifications

• Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent MA-C).
• Minimum 1 year of clinical experience in primary care, hospital, or a specialty healthcare setting.
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills.
• Proficiency in multiple technology platforms and ability to navigate complex systems.
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.
• Experience in care coordination, chronic care management, or behavioral health integration.
• Familiarity with remote patient monitoring (RPM) or remote physiological monitoring platforms.
• Previous experience managing large patient panels (200+).
• Bilingual capability (English and Spanish preferred).

Responsibilities

• Manage an assigned panel of patients and conduct monthly outreach.
• Make 50–60 daily outbound calls to provide care plan support and health coaching.
• Collaborate with clinical teams, providers, and caregivers to identify and address needs.
• Close gaps in patient care through CCM, BHI, and RPM services.
• Document all patient interactions in real time using the care coordination platform.
• Maintain accurate documentation to support care coordination goals.
