Cosán Group is seeking a Care Coordinator for a 100% remote role in the United States. You will manage a panel of patients, provide monthly health coaching, and collaborate with clinical teams to close gaps in care while ensuring HIPAA-compliant documentation.

Qualified candidates hold a CMA/CCMA/CMA or CPhT certification, have at least 1 year of clinical experience, and are proficient with EMR systems. The role requires a private home office and high-speed internet, with a Monday–Friday

Qualifications

• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills.
• HIPAA-compliant home office setup with adequate internet bandwidth.
• Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C) or Certified Pharmacy Technician (CPhT).
• Minimum 1 year of clinical experience in primary care, hospital, or a specialty healthcare setting.

Responsabilités

• Manage an assigned panel of patients and conduct required monthly outreach to ensure continuity of care.
• Conduct 50–60 daily outbound calls to patients, providing care plan support and health coaching.
• Document all patient interactions in real time using the care coordination platform and maintain HIPAA compliance.
