Blehealth, LLC is seeking a Lead Care Manager (LVN) to coordinate care for chronically ill members in a hybrid, mostly remote role serving Pomona, CA. You will work with families, providers, and community resources to implement care plans and promote patient self-management.

Responsibilities include engaging eligible members, coordinating with hospital discharge teams, and advocating for members while ensuring adherence to Medi-Cal MCP guidelines.

Qualifications

• Associate's or Bachelor's degree in health science preferred.
• LVN or Social Worker experience is a plus.
• Must have reliable transportation and own vehicle for site visits.

Responsabilités

• Coordinate with members and providers to ensure seamless services.
• Engage eligible members and oversee ECM services.
• Accompany members to office visits and monitor care plans.
• Connect members with community resources and transportation options.
• Advocate for members with healthcare professionals.
• Educate members and families on care plans and self-management.
